Pennsylvania Youth Leadership Network

The PYLN is a team of youth leaders with disabilities from across Pennsylvania with a purpose to develop the self-determination, empowerment, and leadership of youth that promotes successful post school outcomes in the areas of education, employment, independent living, and health and wellness among youth and young adults throughout Pennsylvania.

Pennsylvania Secondary Transition Guide

The Pennsylvania Department of Education, Bureau of Special Education, and the Pennsylvania Training and Technical Assistance Network (PaTTAN) are pleased to announce the “Pennsylvania Secondary Transition Guide”. A website designed to be a central information hub for youth and families navigating the transition from secondary education to adult life.

The website is organized by secondary transition topics that cover key areas, including post-secondary education and training, employment and community living. Information on the website is designed to highlight important subjects and activities based on a student’s age. The website links to other Pennsylvania agencies and departments that support secondary transition. Materials for youth and families are added to the web portal frequently, so be sure to add the website to your favorites and return to the site often!

Pennsylvania System of Care Partnership

The Pennsylvania System of Care (PA SOC) Partnership is a statewide cooperative agreement and one of many System of Care communities throughout the country. The PA SOC is working to bring youth leaders, family leaders, and system leaders together in equal partnership to integrate the child-serving systems so that desired outcomes are achieved cost effectively through evidenced-based practice and natural supports. Transition Health Care Checklist: Transition to Adult Living in Pennsylvania

The Pennsylvania Department of Health and their community partners recognized a gap in health services for youth with special health care needs. When these youth leave school, they transition from having different or no insurance coverage, from seeing pediatric specialists to searching for adult medical specialists, and from good coverage for medications to having to understand new systems for obtaining medicines. Working with the Pennsylvania Community on Transition State Leadership Team, agencies, and families, the Transition Health Care Checklist: Transition to Adult Living in Pennsylvania was created to guide youth, families, and professionals during this time of change.
