PERFORMANCE OUTCOME MANAGEMENT SYSTEM (POMS) QUARTERLY STATUS UPDATE

If the member presents as a priority population (03, 04, 54, 55) please complete the Quarterly Status Form following the first quarter after the initial encounter. In the following quarters no form needs to be submitted if the status has not changed. If for record keeping, you prefer, you can check “No Change” and fax the form to us. If you are in doubt as to whether to submit a form or not, please do so and VBH-PA will address the issue.

POMS data may be submitted using ProviderConnect, under “Specific Member Search.” ProviderConnect is available online at www.vbh-pa.com/providers.htm.
